Tükürük Bezi Neoplazilerinde Klinik Tecrübemiz; 222 Olgunun Analizi

Öz

TÜRKÇE ÖZET:



Giriş: Tükürük bezi tümörleri(TBT) nadir görülen
kompleks histolojik yapıya sahip tümörlerdir. TBT baş-boyun bölgesinden gelişen
tüm tümörlerinin %3-5 kadarını oluşturur. Tükürük bezi tümörlerinin etiyolojisi
net olmamakla birlikte enfeksiyonlar, travma, sigara ve genetik faktörler
sorumlu tutulmaktadır.



Materyal-Metod: 2006-2018 yılları arasında
tükürük bezi kitlesi ile opere edilen 222 adet olgu incelenmiştir.



Bulgular: Çalışmamızda 222 olgunun 190(%85,5)
tanesi benign tükürük bezi tümörü ve 32 tanesi(14,4) malign tükürük bezi tümörü
olarak sınıflandırılmıştır. Benign tümörlerin %88,2’si parotis, %11,7’si
submandibular bölge yerleşimlidir. Malign tükürük bezi tümörlerinin
%78,1 parotiste, %15,6’sı submandibular bezde, %3,1’i sublingual ve %3,1’i
submental lokasyonda yerleşimlidir.



Sonuç: Tükürük bezi tümörleri oldukça geniş bir
histolojik spektrumu kapsamakta ve tanı açısından güçlük yaratmaktadır. Benign
tümörlerde ayrıntılı histopatolojik değerlendirme malign transformasyon
açısından hayati olabilecektir. Ayrıca yalnızca baş-boyun bölgesinde ortaya
çıkan kitle lezyonlarda değil tüm vücutta primeri bilinmeyen tümörlerde de
karışık histopatolojik görüntü ve uzak organ metastazı yapma kapasiteleri
nedeniyle tükürük bezi tümörlerinin ayırıcı tanıda akılda tutulması önem
taşımaktadır.
